Qualifications

Responsibilities

• Provide support for all Service Coordination Department functions including health education and service coordination services for CCHP members.
• Handle call issue escalation for CCHP members or providers.
• May manage a caseload of members with low to moderate health complexity and assist Team Lead during absences.
