Job summary

Cook Children’s Health Care System in Fort Worth, Texas seeks a bilingual Service Coordinator to support the Clinical Support & Prevention department. You will provide health education and service coordination for CCHP members, and handle call escalations for members or providers.

Ideal candidates have a Bachelor of Social Work, five years in managed care or hospital settings, and must pass a Spanish test. Licensure requirements include CHW and SW credentials.

Qualifications

• High school diploma or equivalent required; Bachelor of Social Work preferred.
• Five years’ experience in a managed care or medical office/hospital environment.
• Bi-lingual (English/Spanish) required. Must pass Spanish test prior to hire.

Education & Experience

• High school diploma or equivalent required. Bachelor of Social Work preferred.
• Five years’ experience in a managed care or medical office/hospital environment.
• Bi-lingual (English/Spanish) Required. Expert proficiency in conversational, reading, writing, and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire.

Knowledge, Skills & Abilities

• Superior verbal and written communication including motivational interviewing and adult learning principles.
• Familiarity with basic medical terminology and ICD-9 and CPT coding.
• Personal computer skills. Expertise with various software.applications/databases (i.e., Microsoft Office).
• Highly organized, ability to prioritize, and demonstrate complex problem solving skills.
• Ability to work autonomously with minimal supervision.
• Demonstrated care coordination concepts.

Licensure, Registration, and/or Certification:

• Licensed Bachelor Social Work preferred.
• Certified Community Health Worker required.
