Job summary

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology or related field.
• Minimum 5 years of community-based experience.
• Current local state driver's license.
• BLS/CPR certification.
• Demonstrate bilingual proficiency in English/Spanish.

Responsibilities

• Coordinates care transitions and ongoing support for high-risk patients across care settings.
• Connects patients with healthcare, social services, community, and government resources.
• Advocates for patients to improve engagement, access to services, and adherence to care plans.
• Collaborates with care teams and community partners to support health and wellness goals.
• Facilitates communication among patients, providers, caregivers, and community organizations to promote coordinated care.
