Job summary

Mercy Health in Ohio is seeking a Community Wellness Navigator to support high‑risk populations through care coordination, patient advocacy, and access to community resources. You will work with providers, community organizations, and government partners to address health needs and social determinants impacting outcomes.

The role promotes continuity of care across settings, strengthens connections to supportive services, and contributes to program evaluation and population health improvement

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology or related field.
• 5 years of community-based experience.
• Bilingual proficiency in English/Spanish.
• Current driver’s license and BLS/CPR certification.

Responsibilities

• Coordinate care transitions for high-risk patients across settings.
• Connect patients to healthcare, social services, and community resources.
• Advocate for patients to improve engagement and access to services.
• Collaborate with care teams to support individualized health goals.
• Document data, report outcomes, and participate in quality improvement.
