You will partner with providers, community organizations, and government agencies to address health and social determinants impacting patient outcomes.

The role emphasizes patient advocacy, continuous care across settings, and participation in program evaluation and quality improvement activities.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology or related field is required.
• 5 years of community-based experience required.
• Licensure: current local state driver’s license; BLS/CPR certification; bilingual proficiency in English/Spanish.

Responsibilities

• Coordinates care transitions and ongoing support for high-risk patients across care settings.
• Connects patients to healthcare, social services, and community resources to address medical and social needs.
• Advocates for patients to improve engagement, access to services, and adherence to care plans.
• Collaborates with care teams and community partners to support individualized health goals.
• Facilitates communication among patients, providers, and community organizations to ensure coordinated care.
