Merakey is seeking a Blended Case Manager for our Philadelphia, PA program. You will develop, coordinate, and implement services for assigned consumers, focusing on behavioral/physical health, family functioning, education goals, and socialization. Qualifications

• Bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or other related human services field.
• Experience in the human services field working with children is preferred.
• Must have a valid driver's license and own reliable transportation.
• MUST BE BILINGUAL.

Responsibilities

• Develop, coordinate, and implement services to assigned consumers as identified in their service plan.
• Assist assigned consumers across life domains: behavioral and physical health, family functioning/independent living, education/vocational goals, and socialization.
• Work with the team to ensure services meet consumer needs and program standards.
