Job summary

Merakey is seeking a Blended Case Manager for our Philadelphia, PA program. You will develop, coordinate, and implement services for assigned consumers, focusing on behavioral/physical health, family functioning, education goals, and socialization. Bilingual candidates are required; a valid driver's license and reliable transportation are needed.

• Experience in the human services field working with children is preferred.
• Must have a valid driver's license and own reliable transportation.

Responsibilities

• Develop, coordinate, and implement services to assigned consumers as identified in their service plan.
• Assist assigned consumers across life domains: behavioral and physical health, family functioning/independent living, education/vocational goals, and socialization.
• Work with the team to ensure services meet consumer needs and program standards.

POSITION OVERVIEW

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.
