Merakey invites applications for a Transition Facilitator/Blended Case Manager in Cambria County, PA. The role develops, coordinates and implements services for consumers, serving as primary contact for up to 30 individuals under supervision.

Qualifications

• Bachelor's degree in sociology, social welfare, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling, or education.
• Or a high school diploma with 12 semester credits in social sciences and two years' direct experience with mental health consumers.

Responsibilities

• Develop and coordinate services per the service plan.
• Provide case-management and supportive counseling to clients.
• Coordinate with other agency providers and handle on-call responsibilities.

Education

Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education

A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers
