Merakey is seeking a Blended Case Manager- Transition Team to join our Cambria County, PA location. The role focuses on developing and coordinating services for up to 30 individuals, with case-management, service planning and coordination with other providers, plus supportive counseling and on-call duties.

The ideal candidate will have a related degree or RN, and the position offers a competitive hourly wage of $19.33 and a comprehensive benefits package.

Qualifications

• Bachelor's degree in sociology, social welfare, psychology or related social sciences or nursing or education.
• RN licensure or equivalent qualification.
• High school diploma plus college credits and mental health experience meet requirements.

Responsibilities

• Develop, coordinate and implement services per the service plan.
• Serve as primary contact for up to 30 individuals.
• Provide case-management, service planning and coordination with other providers.
• Offer supportive counseling and participate in on-call coverage.

Education
