We are seeking a Blended Case Manager to join our team at our program in Philadelphia, PA 19119.
This role is first shift, Monday - Friday 9:00 a.m. - 5:00 p.m.
Earn $24.08/hr.
The Blended Case Management is designed to serve adults living in the community who have serious and persistent mental illness. Using an individualized approach to service delivery, the program strives to promote recovery, independence, and community integration. The Blended Case Manager is responsible for developing, coordinating, and implementing services for adult participants as identified in the goal‑oriented service plan. The role includes providing resource coordination, recovery services, and symptom management using a strength‑based model of case management to 25‑30 adults living with mental illness. The Blended Case Manager will also provide information and support to families and significant others of the individuals served as well as complete required paperwork, correspondence, and chart documentation.
