Qualifications

• One year as an Aging Case Aide 1; or two years of public contact work including one year in a human services program; or any equivalent combination of experience and training.
• This position requires reading, writing, and speaking Spanish. You may need to demonstrate proficiency during the probationary period.

Responsibilities

• Referral Processing: Review referral sources, enter new information, and track consumer status
• Call Screening: Identify presenting issues, gather important details, and direct callers to proper resources
• Eligibility Support: Determine benefit eligibility and assist consumers with forms and applications
• Consumer Guidance: Interview consumers, explain aging programs, and help families make informed choices
• Documentation Management: Record detailed case histories, actions taken, and outcomes for all contacts
• Community Engagement: Participate in wellness fairs and coordinate holiday gift events with partner organizations
