Qualifications

• One year as an Aging Case Aide 1; or two years of public contact work, including one year in a human services program; or any equivalent combination of experience and training.

Responsibilities

• Referral Processing: Review referral sources, enter new information, and track consumer status.
• Call Screening: Identify presenting issues, gather important details, and direct callers to proper resources.
• Eligibility Support: Determine benefit eligibility and assist consumers with completing forms and applications.
• Consumer Guidance: Interview consumers, explain aging programs, and help families make informed choices.
• Documentation Management: Record detailed case histories, actions taken, and outcomes for all contacts.
• Community Engagement: Participate in wellness fairs and coordinate holiday gift events with partner organizations.
