Job summary

The Commonwealth of Pennsylvania announces an Aging Case Aide 2 position in Lehigh County Aging and Adult Services. You will help seniors access programs, gather information, determine eligibility, and guide them to community resources in a supportive team setting.

Work hours are 8:30 AM to 4:30 PM, Monday through Friday. This is a full-time, on-site role with a salary of about $53,955 annually. PA residency and background checks are required for consideration.

Qualifications

• One year as an Aging Case Aide 1; or two years of public contact work in human services; or any equivalent combination of experience and training.
• Must meet PA residency requirements and be able to perform essential job functions.
• Conditional offer requires submission of criminal history reports.
• Veterans' employment preferences apply under PA law.

Responsibilities

• Review and record referrals from calls, mail, faxes, police reports, and other sources.
• Create fliers, calendars, spreadsheets, and other materials for advertising and organizational purposes.
• Screen calls, identify presenting problems, provide information, and collect detailed referral data.
• Determine benefits eligibility and assist consumers with applications for programs such as prescription coverage, tax rebates, housing, and transportation.
• Document all consumer contacts with thorough case histories, resolutions, and dispositions.
• Participate in wellness fairs and coordinate holiday gift events.
