Glia Health Management in Chula Vista, CA is seeking an advanced practitioner (NP/PA) to provide independent clinical assessment and patient care management for neurology patients in an outpatient setting.

You will work with clinic neurologists and NPs, be self-directed, follow standardized procedures, coordinate care, educate patients and families, and participate in meetings. Bilingual English/Spanish preferred; California NP/PA license and DEA required.

Qualifications

• DEA registration required
• Neurology experience preferred
• Bilingual English/Spanish preferred
• Excellent written and verbal communication; ability to thrive in a fast-paced environment

Responsibilities

• In collaborative practice with the clinic neurologists and NPs, the Nurse Practitioner/ Physician Assistance is responsible for providing independent clinical assessment and patient care management services for Neurological patients.
• The NP/PA must be self-directed and function with minimal direct supervision. The NP/PA functions in an interdependent role by implementing the appropriate Standardized Procedures (SPs).
• The NP/PA will provide and coordinate the care of patients, provide patient/family education, promote safe continuity of care. The NP also participates in clinical meetings, provides education and support, as well as makes appropriate referrals.
• Document services rendered.

• Previous experience in neurology preferred
