Job summary

Glia Health Management's San Marcos, CA clinic is seeking an advanced practitioner (NP/PA) to provide high-quality patient care in an outpatient neurology setting. You will work with neurologists and NPs, perform independent assessments, and coordinate care with education for patients and families.

Requirements include California NP/PA license and a DEA; previous neurology experience and bilingual English/Spanish are preferred.

Qualifications

Responsibilities

• Provide independent clinical assessment and patient care management for neurological patients in collaboration with clinic neurologists and NPs.
• Self-directed NP/PA work with standardized procedures and minimal supervision.
• Coordinate patient care, educate patients/families, participate in clinical meetings and make appropriate referrals.
• Document services rendered.

Responsibilities:

• In collaborative practice with the clinic neurologists and NPs, the Nurse Practitioner/ Physician Assistance is responsible for providing independent clinical assessment and patient care management services for Neurological patients.
• The NP/PA must be self-directed and function with minimal direct supervision. The NP/PA functions in an interdependent role by implementing the appropriate Standardized Procedures (SPs).
• The NP/PA will provide and coordinate the care of patients, provide patient/family education, promote safe continuity of care. The NP also participates in clinical meetings, provides education and support, as well as makes appropriate referrals.
• Document services rendered.
