Job summary

Prime Healthcare St Joseph Hospital in Joliet, IL, is seeking a Mental Health Counselor to provide emotional support, crisis intervention and counseling to patients and families. The role includes psychosocial assessments, treatment planning, and discharge coordination within an interdisciplinary team.

You will perform documentation and maintain clinical records; inpatient or outpatient behavioral health experience is preferred.

Qualifications

• Bachelor’s degree in Social Work, Marriage and Family Therapy or other related required.
• Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor or related field, preferred.
• Current registration an intern with Board, as appropriate to education and licensing requirements.
• Current BLS certificate upon hire and maintain current.
• Minimum one (1) year Inpatient or Outpatient Behavioral Health experience preferred.

• Schedule: Full-Time, Nights, 7p-7a, every other weekend and rotating holidays.
• Facility: Prime Healthcare St Joseph Hospital
• Location: Joliet, IL 60435

*This is a Mental Health Counselor position.

Responsibilities

To provide emotional support, crisis intervention and counseling; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to treatment services, illness, disability or trauma. To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of an interdisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.
