The role focuses on assessment, care coordination, brief evidence-based interventions, patient education, and ongoing symptom monitoring to improve outcomes and patient experiences.

Qualifikationen

• Masters degree or higher in Social Work, Counseling, or a related behavioral health field.

• Coordinate behavioral health care with primary care providers and specialists.
• Screen and assess patients for mental health and substance use disorders.
• Provide patient education on behavioral health conditions and treatments.
• Monitor symptoms and treatment response via in-person and telehealth visits.
• Support psychotropic medication management and adherence.
• Deliver brief evidence-based behavioral health interventions (e.g., Behavioral Activation).
• Document patient encounters and track outcomes.
