Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Behavioral Health Specialist to join its Behavioral Health Collaborative Care team. You will assess, coordinate care with primary care providers and psychiatric consultants, provide brief evidence-based interventions, educate patients, and monitor symptoms to improve outcomes.

Qualifications

• Master's degree or higher in Social Work, Counseling, or related field.
• Valid Michigan clinical licensure (LMSW, LPC, LMFT, or similar).
• Experience with CBT, PST, or Behavioral Activation preferred; experience with co-occurring conditions preferred.

Responsibilities

• Assess and coordinate behavioral health care with primary care providers and specialists.
• Screen and assess patients for common mental health and substance use disorders.
• Educate patients on behavioral health conditions and treatments.
• Monitor symptoms, treatment response, and medication side effects via in-person and telehealth.
• Support psychotropic medication management and adherence.
• Deliver brief evidence-based behavioral health interventions.
• Track patient outcomes and document encounters.
