Oak Street Health seeks a Behavioral Health Specialist (LCSW/LCPC/LMFT) who is bilingual in Spanish to provide care management and short-term psychotherapy within our primary care clinics, coordinating with care teams and a psychiatric consultant to address behavioral health needs.

The role supports patient assessments, referrals, warm handoffs for telehealth, and accurate EMR documentation. Remote days may be possible based on metrics, with a 40-hour week and competitive benefits.

Qualifications

• Master's degree in mental health counseling/healing arts from an accredited program.
• Highest clinical license level or local equivalent.
• US work authorization.
• Experience with screening for mental health or substance use disorders.
• Experience with assessment and treatment planning.

Responsibilities

• Collaborate with primary care teams to identify patients for referral.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants.
• Provide short-term counseling and evidence-based treatments.
• Document encounters accurately in the EMR.
• Participate in regional efforts to advance the Behavioral Health Program.
• Possibly covering multiple clinics as needed.
