Job summary

I Am Boundless is seeking a Care Coordinator to assess needs, coordinate care, and link services for youth and families enrolled in the OhioRISE program. The role is community-based and may include some tasks from home, engaging with families, caregivers, and service providers through face-to-face and virtual channels.

The position requires a Bachelor’s degree in psychology, social work or related field, or at least three years of relevant experience in children’s behavioral health or public

Qualifications

• Bachelor’s degree or 3+ years experience in children's behavioral health or related field.
• High School diploma or GED required.
• Experience in family, community and case management related areas listed above.

Responsibilities

• Assess needs, link services and resources, and coordinate care for youth and families under the OhioRISE plan.
• Engage with individuals, parents/guardians, Boundless staff and providers via in-person, phone, video, and electronic communication.
• Schedule meetings, track documentation, follow up on needs and appointments, and report outcomes.
