Job summary

Oak Street Health, in partnership with CVS Health, seeks a Behavioral Health Specialist to deliver care management and short-term psychotherapy across regions. You will collaborate with primary care teams, assess patients, and coordinate with psychiatric consultants and external resources.

The role requires a master’s degree in a mental health field and licensure at the highest clinical level, with a strong emphasis on evidence-based, brief interventions and patient-centered care.

Qualifications

• Master's Degree in mental health counseling, social work, or related field.
• License at the highest clinical level (LCSW/LPC/LMFT) or local equivalent.
• Experience screening for mental health or substance use disorders.
• Experience with assessment and treatment planning.
• Familiarity with brief, structured interventions (MI, Behavioral Activation, CBT).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to refer patients to Oak Street Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants and telehealth providers.
• Document encounters accurately in the EMR.
• Support regional/organizational efforts to improve the program.
• Possibly cover multiple clinics as needed.
