Rockford Center in Newark, DE, a 138-bed behavioral health facility, seeks a Social Worker to support patients and families across the care continuum. You will conduct psychosocial assessments, help develop discharge plans, connect patients with community resources, and advocate for services that promote recovery.

Qualificações

• Master's Degree in Social Work or related program; license eligible.
• LPCMH/LCSW preferred.
• Previous experience in behavioral health setting preferred.
• Current CPR Certification.

Responsabilidades

• Conduct psychosocial assessments to document patient history and identify treatment focus.
• Develop and implement treatment plans; ensure concise, realistic care plans.
• Provide individual, group and family therapies using various modalities.
• Document treatment provided to patient in medical record.
• Develop and coordinate an individualized discharge plan with the treatment team.
• Identify community resources to meet aftercare needs.
• Participate in regular treatment team meetings and coordinate the case with the team.
