Job summary

Mercyhealth in Rockford, IL is seeking a Case Management Social Worker to coordinate discharge planning, assess psychosocial needs, and connect patients and families with community resources. You will collaborate with physicians and the care team to ensure appropriate utilization of services and smooth transitions of care.

Qualifications include a BS W or equivalent, LSW/LCSW in state, 1–2 years acute care experience; proficiency with Allscripts Care Management and Epic EMR; eligible for on-call

Qualifications

• Bachelor’s degree in social work (BSW) or equivalent is required.
• LSW or LCSW in state of practice is required.
• 1–2 years of related experience in acute care is desirable.
• Case Management Certification (ACMA) preferred.

Responsibilities

• Provides psychosocial assessment and intervention for hospital patients.
• Develops and implements psychosocial treatment plans.
• Assesses discharge planning needs and barriers to transition.
• Maintains plan of care documentation in the medical record.
• Facilitates crisis intervention and community resource referrals.
• Coordinates with multidisciplinary team to ensure patient rights and support.
