Job summary

ChristianaCare in Newark, Delaware, is seeking a skilled social worker to assess patients and families, develop discharge plans, and provide counseling to support coping with illness and transitions. You will collaborate with the care team to ensure safe, timely discharges and coordinate guardianship or crisis interventions when needed.

The role requires an MSW/MSS and appropriate DE licensure, with acute care experience preferred. A CCM certification is desirable within 18 months of eligibility.

Qualifications

• Master’s degree in social work (MSW) or Master of Social Services (MSS) from an accredited graduate school.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.
• Maternal-Child position requires experience with neonates, children and families.

Responsibilities

• Conducts psychosocial assessment of patients and families related to social, emotional, and cognitive needs.
• Develops and implements intervention plans addressing identified needs.
• Collaborates with the multidisciplinary team to ensure appropriate discharge planning.
• Provides counseling and interventions to assist with coping with new diagnoses, chronic illness, or end-of-life care.
• Advocates for patients to obtain necessary services.
• Coordinates family conferences to facilitate patient care progression.
• Manages cases involving abuse/neglect, domestic violence, guardianship, foster care, or advance directives.
• Facilitates completion of placement documentation and referrals to options programs.
• Liaises with Care Managers to provide discharge planning per departmental policies.
