Job summary

Universal Health Services in Newark, DE, seeks a Social Worker for our Partial Hospitalization Program. This full-time role supports adults with psychosocial care, coordinating discharge plans and linking patients to community resources within a behavioral health hospital setting.

The position emphasizes collaboration with the treatment team to address psychosocial and environmental factors affecting recovery, with a Monday–Friday schedule and LMSW requirements. RN supervision available.

Qualifications

• Master's Degree in Social Work from an accredited college (must be license eligible).
• LMSW required
• Previous experience in behavioral health setting preferred
• Current CPR Certification

Responsibilities

• Conduct psychosocial assessments to document patient history and identify issues for treatment focus by interviewing the patient and family.
• Participate in development and implementation of treatment plans with concise, realistic care plans.
• Provide individual, group and family therapies using various modalities.
• Document treatment provided in the medical record legibly and timely.
• Develop and coordinate an individualized discharge plan with the treatment team.
• Identify and assess family or community resources to meet aftercare needs.
• Attend treatment team meetings to coordinate case management and provide social work perspective.
