Job summary

ChristianaCare in Newark, DE is seeking a social worker to assess and enhance patients' coping during illness and discharge planning. You will collaborate with the multidisciplinary team to develop and implement discharge plans and to provide counseling and advocacy.

Qualifications

• Master’s degree in social work (MSW) or MSS from an accredited graduate school.
• Licensed CSW or MSW as required by Delaware.
• Acute Care Social Work experience preferred.
• Maternal-Child position requires experience working with neonates, children and families.

Responsibilities

• Conducts psychosocial assessment/reassessment of patients and families.
• Develops and implements intervention plans.
• Collaborates with the multidisciplinary team for discharge planning.
• Provides counseling and advocacy related to coping with illness and care needs.
• Facilitates family conferences and coordinates with Care Managers.
