Job summary

You will develop individualized care plans with patients and designated care partners, collaborate with inter-professional teams, use IT to monitor outcomes, participate in safety and quality committees, and pursue ongoing professional growth as a preceptor.

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• Knowledge of patient- and family-centered care in care planning.
• Cultural awareness and respectful engagement with diverse patients.
• Strong communication: transmitting and interpreting information with patients and families.
• Knowledge to assess ailments and implement a plan of care.
• Education of patients and care partners on condition, treatment, and next steps.
• Medication administration and knowledge of prescribed drugs.
• Discharge planning and arranging follow-up care.
• Knowledge of assessment, diagnosis, and clinical judgement.
• Documentation of health information accurately in records.
• BLS certification and RN licensure in NJ.
