Qualifications

• Effective communication concepts, tools, and techniques to engage patients and families.
• Knowledge of discharge planning and coordination of follow-up care.
• Ability to assess patient conditions, develop care plans, and implement treatments.
• Experience with health information documentation and data use in care.
• Ability to collaborate within nursing and inter-professional teams.
