Job summary

Elara Caring is seeking a Behavioral Health Registered Nurse (RN) for home health in the Westlake, Lorain, and Elyria area of Ohio. You will deliver one-on-one nursing care in patients’ homes, coordinating with a clinical team to meet their unique needs.

This role emphasizes autonomy, flexible scheduling, and ongoing opportunities to grow technical and clinical skills within a supportive home health network. Travel within the Cleveland west side is required.

Qualifications

• Current, unrestricted RN license in Ohio.
• At least 1 year of nursing experience in a clinical care setting.
• Willingness to travel throughout the assigned territory (approx. 50%).
• Dependable vehicle, valid driver’s license, and auto insurance per state laws.
• Ability to frequently lift, push, pull, and transfer patients up to 50 pounds.

Responsibilities

• Assess patients’ physical, behavioral health, psychosocial, and environmental needs.
• Manage assigned caseload by assessing, planning, coordinating, delivering, and evaluating nursing care.
• Reassess patients during visits and update care plans as needs change.
• Educate patients, families, and caregivers on following the care plan and managing symptoms.
• Communicate significant findings and safety concerns to the supervisor, physician, and care team.
• Coordinate with other providers and care settings to support continuity of care.
• Supervise and document BH aide and LPN care per policy.
• Maintain accurate clinical documentation.
• Provide support at time of death and complete bereavement assessments when applicable.
