Job summary

Elara Caring is seeking a Registered Nurse in Behavioral Health to provide compassionate, patient-centered care across East Cuyahoga, Lake Geauga, and Ashtabula counties. You will assess, plan, implement, and evaluate nursing care for behavioral health patients, coordinating with an interdisciplinary team to deliver high-quality home-based care.

The role requires an active RN license, 1+ year clinical nursing experience, and willingness to travel up to 50% within the branch area.

Qualifications

• Current, unrestricted state RN license
• 1+ years of clinical nursing experience
• Willing and able to travel within branch/office coverage area
• Dependable vehicle, valid driver’s license and current auto insurance
• Ability to frequently lift, push, pull, and move up to 50 pounds
• Associates or Bachelors in Nursing preferred
• Home health/home care experience preferred

Responsibilities

• Assess caseload of behavioral health patients and families to identify needs
• Manage caseload planning, coordination, implementation and evaluation of care
• Gather data on physical, psychological, social, and spiritual factors
• Perform initial evaluations and re-evaluations of nursing needs
• Conduct follow-up visits as required by patient, family, and facility
• Communicate significant findings to supervisor, physician, and care team
• Revise care plan based on identified issues and notify team
• Counsel and educate patient, family, and facility in the plan of care
• Teach behavioral health concepts and pain/symptom management to staff in alternate settings
• Initiate and supervise delivery of care by aides and LPNs per policy
• Notify supervisors of unusual or problematic issues
• Provide bereavement support when appropriate
