Job summary

Elara Caring is seeking a Behavioral Health Registered Nurse for Home Care on a PRN basis with a flexible schedule and consistent weekly availability. Join a team delivering personalized, one-on-one care in patients’ homes, away from large facility shifts.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Associate or bachelor’s degree in nursing.
• At least one year of clinical nursing experience.
• Behavioral health, home health, or hospice experience preferred.
• Homecare HomeBase experience preferred.

Responsibilities

• Assess patients’ physical, behavioral, psychosocial, and environmental needs.
• Develop, coordinate, implement, and evaluate individualized plans of care.
• Manage an assigned caseload and complete initial and ongoing nursing assessments.
• Monitor patients for changes in condition, safety concerns, and barriers to treatment.
• Communicate important updates to physicians, clinical leadership, caregivers, and other members of the care team.
• Educate patients and caregivers about medications, symptom management, safety, and treatment goals.
• Update plans of care based on patients’ changing needs and progress.
• Coordinate services with an interdisciplinary team to support positive patient outcomes.
• Supervise and document care provided by LPNs and behavioral health aides when applicable.
• Maintain accurate and timely clinical documentation.
