Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health to provide in-home visits across our service area. You will assess, plan, implement, and evaluate care for behavioral health patients in home settings, ensuring compassionate, high-quality care tailored to each patient’s needs.

You will report to the Clinical Manager and work in a supportive, autonomous environment with flexible schedules and opportunities for education.

Qualifications

• Current, unrestricted state RN license
• 1+ year of clinical nursing experience
• Willingness to travel within branch/office area
• Dependable vehicle and valid driver’s license with current auto insurance
• Ability to lift/push/pull up to 50 pounds and transfer patients
• Willingness to travel 50% of the time
• Associates or Bachelors in Nursing preferred
• Home Health/Hospice experience preferred
• Must have access to reliable transportation to travel between patient homes

Responsibilities

• Assess caseload of behavioral health patients and families to identify needs.
• Lead the plan of treatment from assessment through evaluation.
• Document data affecting health status and care plans.
• Perform initial evaluation and follow-up visits.
• Communicate significant findings to the clinical team.
• Revise care plans as needed and coordinate with staff.
• Educate patients and families on care plans and pain management.
• Supervise and document care delivery by aides and LPNs.
• Provide support at time of death and bereavement assessment.
