Elara Caring is seeking a Behavioral Health Registered Nurse (RN) for home health in Westlake, Lorain, Elyria and surrounding areas. The role emphasizes one-on-one patient care in patients' homes, with travel across the Cleveland west side communities.

• At least 1 year of nursing experience in a clinical care setting.
• Ability and willingness to travel throughout the assigned territory, with approximately 50% travel.
• A dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Preferred: Home health or hospice experience; Homecare Homebase experience.

Responsibilities

• Assess patients’ physical, behavioral health, psychosocial, and environmental needs.
• Manage assigned caseload, including planning, coordinating, providing, and evaluating nursing care.
• Reassess patients during visits and update care plans as needed.
• Educate patients and families on the care plan and symptom management.
• Coordinate with other providers to support continuity of care.
