Job summary

Elara Caring seeks a Behavioral Health Registered Nurse to deliver one-on-one nursing care across home health settings in Ohio. You will assess patients, plan care, and coordinate with a clinical team to ensure continuity of care in the patient’s home environment.

The role emphasizes autonomy, frequent travel across the West Side of Cleveland’s communities, and ongoing professional development. A valid Ohio RN license and 1+ year of clinical nursing experience are required.

Qualifications

• Current, unrestricted RN license valid in Ohio.
• At least 1 year of nursing experience in a clinical care setting.
• Ability and willingness to travel throughout the assigned territory, with approximately 50% travel.
• A dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Assess patients’ physical, behavioral health, psychosocial, and environmental needs.
• Manage your assigned caseload, including assessing, planning, coordinating, providing, and evaluating nursing care.
• Reassess patients during visits and update care plans as needs change.
• Teach patients, families, and caregivers how to follow the care plan and manage symptoms.
• Communicate significant findings, changes in condition, and safety concerns to the clinical supervisor, physician, and care team.
• Coordinate with other providers and care settings to support continuity of care.
• Supervise and document behavioral health aide and LPN care according to agency policy.
• Maintain accurate clinical documentation.
• Provide support at the time of death and complete bereavement assessments when applicable.
