Elara Caring is seeking a Behavioral Health RN to deliver personalized home-based care across Eastern Cuyahoga, Lake, Geauga, and Ashtabula counties in Ohio. You’ll build meaningful patient relationships while maintaining independence, backed by a collaborative clinical team.

Qualifications

• Current, unrestricted Ohio RN license.
• At least one year of clinical nursing experience.

• Ability to frequently lift, push, pull, and assist patients up to 50 pounds.

Responsibilities

• Assess patient needs, manage an assigned caseload, and develop and update care plans.
• Provide nursing care and educate patients and caregivers about treatment and symptom management.
• Coordinate with providers and the clinical team, reporting changes in condition and safety concerns.
• Supervise LPN and behavioral health aide services as required and maintain timely documentation.
