Job summary

Elara Caring in Michigan is seeking a Registered Nurse – Behavioral Health to deliver compassionate, high-quality home-based care to patients and families in their preferred setting. You will assess needs, plan care, coordinate with an Interdisciplinary Team, and document ongoing progress during each visit.

This role emphasizes 1:1 patient care, travel within branch coverage area, and clear communication of health status changes to supervisors and physicians.

Qualifications

• Current, unrestricted RN license.
• 1+ year clinical nursing experience.
• Able and willing to travel within branch/office coverage area.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to frequently lift up to 50 pounds and assist with transfers.
• Home Health/Hospice/HomeCare HomeBase experience preferred.

Responsibilities

• Assesses caseload of behavioral health patients and families to identify needs.
• Assumes primary responsibility for caseload planning and evaluation of care.
• Assesss data on physical, psychological, social, and spiritual factors and incorporate into plan of care.
• Performs initial evaluation and re-evaluation of nursing needs during visits.
• Communicates significant findings to Clinical Supervisor/physician and team.
• Revises plan of care as issues are identified and notifies team.
• Counsels and educates patient and family in the Interdisciplinary Plan of Care.
• Teaches behavioral health principles to staff in alternate care settings.
• Initiates and documents delivery of care per agency policy.
• Informs supervisor of any unusual facility issues and provides support at time of death.

We are seeking a Registered Nurse – Behavioral Health with strong clinical skills, commitment, and compassion.

As a Registered Nurse Behavioral Health, you’ll contribute to our success in the following ways:

• Assesses assigned case load of behavioral health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.


• Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.


• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.


• Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs.


• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.


• Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.


• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.


• Teaches behavioral health philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).


• Initiates, supervises, and documents observance of the behavioral health aide and LPN delivery of care per agency policy.


• Informs supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues.


• Provides appropriate support at time of death and perform bereavement assessment.

Requirements

• Current, unrestricted RN license


• 1+ year clinical nursing experience


• Able and willing to travel within branch/office coverage area


• Valid driver’s license, reliable vehicle, and current auto insurance


• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment


• Associate’s or Bachelor’s in Nursing preferred


• Home Health, Hospice, or HomeCare HomeBase experience preferred
