Job summary

Crossroads Health in Mentor, Ohio, seeks a licensed prescriber to deliver psychiatric assessment and medication management within outpatient Youth and/or Adult Psychiatry in the Health Center Program. The role emphasizes trauma-informed care and collaborative team participation to support recovery and integrated services.

Qualifications

• Master's degree (minimum) in a relevant field.
• Licensed in Ohio as PA/NP/APRN.
• Experience in mental health treatment.
• CS Registration in Ohio.
• Experience in hospital or family practice primary care APP (1 year preferred).
• Professional liability insurance (employer provided).
• BLS/First Aid certification (employer provided).

Responsibilities

• Provide trauma-informed pharmacological management to patients in office or virtually.
• Evaluate and manage medications for behavioral health conditions; monitor and manage side effects.
• Provide delegation, direction and supervision to Integrated Care staff as appropriate.
• Document psychiatric evaluations, follow-up notes, medication orders, lab requisitions, and related documents.
• Collaborate with community partners and patient treatment teams; supervise nursing and MA staff as appropriate.

Come and be a part of our Integrated Health Center with locations in Mentor and Lake County. The Prescriber offers psychiatric assessment and medication management within the outpatient Youth and/or Adult Psychiatry setting in the Health Center Program. This position is responsible for individual services to patients assigned by Crossroads Health. This licensed provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health.

Additionally, the Prescriber will:

• Provide trauma informed Pharmacological Management to individual patients in the office setting, or virtually, including psychiatric assessment of behavioral or physical health symptoms, rendering diagnoses as appropriate utilizing DSM and ICD criteria and codes.
• Evaluation and management of medications to treat behavioral health conditions, monitoring and managing side effects, ordering medications as appropriate consistent with federal, state and organizational policies.
• Provide delegation, direction and supervision as appropriate to the Integrated Care RN, Integrated Care LPN, Integrated Care MA and the Integrated Care Office Manager as appropriate.
• Provide accurate, concise, and concurrent documentation according to program procedures and policies. Documentation will include psychiatric evaluations and follow-up progress notes, entering all medication orders, laboratory requisitions and any additional documents as required.
• Provide consultation and collaboration with community partners and patients' treatment teams. Provide direction, delegation and supervision of pagtient care to the nursing and MA staff with Crossroads Health.

Position Requirements:

• Master's Degree (minimum)
• Experience in mental health treatment preferred
• Licensed in the State of Ohio as a Physician Assistant, Nurse Practitioner, Advanced Practice Registered Nurse
• Experience in a hospital or family practice setting as a primary care APP (one year preferred)
• Controlled Substance Registration License in Ohio
• Professional Liability Insurance. (employer provided)
• Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable) (employer provided)
• BLS/First Aid (employer provided)
