Job summary

Crossroads Health in Northeast Ohio seeks a Case Manager to provide in-home or community-based therapeutic interventions, link clients to resources, assess needs, and advocate on their behalf.

The role requires a High School Diploma with 3 years in behavioral/mental health, QMHS/QBHS within 15 days, strong organization, reliable transportation, and a commitment to equity and client-centered care. The role includes meeting monthly productivity metrics and a drug-free workplace.

Qualifications

• High School Diploma with mandatory 3 years experience in behavioral/mental health.
• QMHS/QBHS within 15 days of hire (employer provided).
• Demonstrated passion to advocate on behalf of others.
• Demonstrated self-starter and independent thinker who can work in the field as well as in the office.
• Organized with good time management skills necessary.
• Must have an acceptable driving record 4 (four) points or less, reliable transportation and carry automobile insurance with minimum liability of $100,000/$300,000, and property damage of $50,000.

***The anticipated hiring range for this position is $20.00 - $23.00 per hour. Starting pay will be commensurate with experience, qualifications, and internal equity.
