AdventHealth in Orlando is seeking a Behavioral Health Social Worker to support patients and families through discharge planning and community integration. You will collaborate with a multidisciplinary team to document psychosocial determinants of health, conduct psychosocial assessments and provide psychotherapy as needed.

The role involves crisis management, participation in physician rounds, and linking to community resources to avoid unnecessary readmissions.

Qualifications

• Master's in Social Work or Master’s in Mental Health Counseling required.
• LCSW or LMHC licensure required.
• 2+ years of clinical social work experience preferred in acute care.
• Experience with Baker Acts and Marchman Acts preferred.

Responsabilités

• Maintains community resources and networks to assist patients and families during discharge.
• Collaborates with the multi-disciplinary team to document psychosocial determinants of health for safe discharges.
• Provides psychosocial assessments and psychotherapy with patients and families.
• Attends rounds to provide behavioral health input to the team.
• Follow-ups post-discharge to ensure compliance with the plan.
