Ideal candidates bring a Bachelor’s degree in Social Work, Counseling, or related field, nursing licensure if applicable, and prior psychiatric mental health experience.

Qualifications

• Bachelor’s degree in Social Work, Clinical/Counseling, or Social Science related area required
• Graduate of an accredited nursing program
• Experience with psychiatric mental health population preferred
• Licensure as RN if applicable

Responsibilities

• Provide discharge planning and case management services within hospital
• Coordinate with referral sources and hospital departments
• Support crisis intervention and liaison activities as needed
