Qualifications

• Licenses and Certifications: LCSW in North Dakota.
• Educational Requirements: Master’s degree or above.
• Experience Requirements: Requirements for LCSW licensure are completed.
• Special Skills or Training Requirements: As determined by the Department Manager or needs of clinic.

Responsibilities

• Perform psychosocial assessments to identify patient needs, strengths, barriers, and available support systems.
• Participate in the development and implementation of care plans, collaborating with treatment teams to support patient stabilization and recovery.
• Assist patients and families with discharge planning, service referrals, and connection to community resources and support agencies.
• Provide crisis intervention, supportive counseling, and patient education within the scope of practice for a Licensed Baccalaureate Social Worker.
• Document assessments, care coordination activities, and patient interactions accurately and timely in the electronic health record.
• Maintain effective communication with interdisciplinary staff, external agencies, and family members to promote continuity of care and safe transitions.
