Responsibilities

• Educates patients and families about illness impacts and mobilizes resources while advocating for patient empowerment.
• Assesses readmitted patients for reasons and collaborates on care plans.
• Organizes and facilitates discharge planning conferences with the care team.
• Documents discharge plans, barriers, and progress in the medical record.
• Advocates for patient rights and coordinates post-acute care with payors as needed.
• Provides grief counseling, crisis intervention, and care planning support.
• Reviews records to align clinical and social factors for timely discharge.
