Job summary

ADP, Inc. in Georgia seeks a Behavioral Health Medical Navigator / PP-Case Management Contractor to provide targeted case management and healthcare navigation for individuals with mental health and co-occurring needs.

You will collaborate with clinical teams to identify clients who may benefit from enhanced navigation, coordinate access to medical, behavioral health, and specialty-care services, and support continuity of care while maintaining confidentiality.

Qualifications

• Bachelor’s degree in social work, psychology, health education, or related helping field or equivalent experience.
• Experience in behavioral health, case management, healthcare navigation, social services, or related field.
• Demonstrated knowledge of community-based resources and healthcare systems.

Responsibilities

• Review assessments, treatment plans, referrals to identify navigation needs.
• Coordinate access to medical, psychiatric, behavioral health, and specialty-care appointments.
• Facilitate referrals and linkages to providers and community resources.
• Provide follow-up and care coordination after hospital or emergency care.
• Protect client confidentiality and maintain professional boundaries.
