Job summary

Centene Corporation is hiring for a Behavioral Health Care Manager supporting foster care members in NW Kansas. The role involves developing and facilitating complex care management activities, including behavioral health and social determinants needs, with up to 75% local travel.

Remote work from home is available for the remaining time, and the schedule is Monday–Friday. Requires a Master’s degree in Behavioral Health or related field and 2–4 years of experience.

Qualifications

• Master's degree in Behavioral Health or Social Work or a nursing degree with BH experience.
• 2–4 years of related experience requirement.
• Licensed Master Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements.

Responsibilities

• Evaluate member needs via phone or in-home visits to develop care/service plans.
• Perform telephonic, digital, home or site visits outreach to assess needs.
• Develop care plans for high-acuity members and coordinate providers and community resources.
• Coordinate between members, families, caregivers and care team to ensure access to services.
• Monitor plans and outcomes and adjust based on member needs.
• Provide education to members and families on procedures, treatments, and benefits.
• Comply with policies and standards.
