CareSource is seeking an Integrated Care Clinical Manager - BH in Massachusetts to provide monitoring and clinical care management for dually eligible enrollees with complex needs. The role focuses on integrating health services with community resources to improve outcomes and quality of life.

The position requires a Master’s degree in social work or mental health counseling with an independent license, plus experience in care coordination or dual-eligible populations.

Competenze

• Master's degree in social work or mental health counseling and independent license required.
• BH Clinician with ability to independently serve people with complex medical, behavioral and social needs.
• Medicaid/Medicare managed care experience preferred.

Mansioni

• Engage enrollees in home and community settings to build complex care management relationships.
• Coordinate with ICT to create holistic care plans addressing medical and non-medical needs.
• Lead the interdisciplinary care team and collaborate with internal and external stakeholders.
• Oversee enrollee utilization of long-term services and supports.
• Advise members on benefits and services under Medicare and Medicaid.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
