Job summary

CareSource Massachusetts is seeking an Integrated Care Clinical Manager - BH to provide monitoring, follow-up and clinical care management to dually eligible enrollees with complex medical, behavioral and social needs in community settings. The role requires licensure (LCSW/LISCW/LMHC), a Master’s in social work or mental health counseling, and strong coordination across providers.

Occasional travel within the state is required as you partner with primary care, behavioral health specialists, and

Qualifications

• Master’s degree in social work or mental health counseling.
• Independent license (LCSW/LISCW/LMHC) required.
• Experience in care coordination or case management with dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.
• Must have valid driver’s license and access to a vehicle.

Responsibilities

• Engage enrollees in homes and community settings to build complex care management relationships.
• Coordinate with health providers and community resources to ensure seamless care transitions.
• Conduct comprehensive assessments and functional assessments as needed.
• Lead the interdisciplinary care team to develop holistic care plans.
• Monitor enrollee utilization of long-term services and supports.
• Provide education on benefits, medications, and preventive care.
• Travel to member, provider and community visits as required.
