Job summary

Elara Caring is seeking a Registered Nurse with behavioral health experience to cover home health visits in the Jackson area. You will manage a caseload, coordinate with the care team, and provide patient and family education, while traveling within the assigned territory.

The role offers autonomy, flexible scheduling, comprehensive benefits, and opportunities for professional growth. Prior home health and behavioral health experience strongly preferred.

Qualifications

• At least 6 months to 1 year of Behavioral/Psych nursing, preferably in inpatient
• Qualified candidates have RN license and home health exposure preferred
• Willingness to travel within branch/office coverage area and 50% travel requirement

Responsibilities

• Assess caseload of behavioral health patients to identify needs
• Manage plan of care, including evaluation and adjustments
• Coordinate care with interdisciplinary team and physicians
• Provide education and counseling to patients and families
• Document and communicate patient status and changes to supervisors
