Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health to provide in-home visits. This role emphasizes 1:1 patient care, planning and evaluating the care plan, and coordinating with the care team in patients' homes.

Requires an RN license and 1+ year of nursing experience, with travel up to 50%. A valid driver’s license and reliable transportation are essential for community-based care in various neighborhoods.

Qualifications

• Current, unrestricted RN license in the state of practice.
• 1+ years of clinical nursing experience.
• Willingness to travel within the coverage area and maintain reliable transportation.

Responsibilities

• Assess assigned behavioral health patient caseload and coordinate care.
• Manage the plan of care, including assessment, planning, and evaluation during visits.
• Provide 1:1 patient care and educate patients and families.
• Communicate findings to supervisors and care team; support Bereavement assessment when needed.
• Travel to patient homes up to 50% of time; lift and position patients as required.
