Job summary

Elara Caring in Springfield seeks a Registered Nurse Behavioral Health to provide in-home visits across the Springfield area. This full-time role emphasizes autonomy, compassionate care, and delivering the right care at the right time in the patient’s home.

You will assess patients, plan and implement care, and collaborate with the clinical team. Travel up to 50% is expected; a valid driver’s license and reliable vehicle are required; a comprehensive benefits package is included.

Qualifications

• Current, unrestricted state RN license
• 1+ years of experience in a clinical care setting as a nurse
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must be able and willing to travel 50%
• Associates Degree or Bachelors Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with Homecare HomeBase is preferred
• You will report to the Clinical Manager or Clinical Supervisor

Responsibilities

• Assesses assigned patient loads to identify physical, psychosocial, and environmental needs.
• Assesses patient needs and obtains data for the plan of care.
• Makes initial evaluations and adjusts nursing plan of care.
• Performs follow-up visits according to patient and facility needs.
• Communicates findings to Clinical Supervisor and care team.
• Teaches and counsels patients and families on plan of care.
