Job summary

Elara Caring seeks a Registered Nurse (Behavioral Health) to provide compassionate home-based care Fridays and weekends. You will assess patient needs, implement and adjust plans, and educate patients and families in their home environment.

This role emphasizes autonomy, 1:1 patient care, and collaboration with the Clinical Manager and care team. Ideal candidates have an unrestricted RN license, 1+ year in clinical care, and willingness to travel 50% within the branch area.

Qualifications

• Current, unrestricted state RN license.
• 1+ year of experience in a clinical care setting.
• Willingness to travel within branch area and have dependable vehicle and valid driver’s license.
• Able and willing to travel up to 50%.
• Associates or Bachelor’s in Nursing preferred.
• Home Health or Hospice experience preferred.
• Experience with Homecare HomeBase preferred.

Responsibilities

• Assess caseload of behavioral health patients and families to identify needs.
• Manage caseload planning, coordination, implementation and evaluation of care plan.
• Evaluate patient nursing needs during each visit and follow up as required.
• Communicate significant findings to supervisor/physician and care team.
• Revise care plans as needed and educate patient and family on care.
• Provide support at time of death and conduct bereavement assessment.
